Safe Prescribing

Evaluate the patient: The medical record must contain a complete history and physical examination documenting the nature and intensity of the pain as well as co-existing conditions.
Treatment Plan: Objectives should state objectives that will be used to determine treatment success, such as pain relief, an/or improved physical or psycho-social function.
Informed Consent and Agreement for Treatment: Discuss the risks and benefits to the use of controlled substances with the patient, appropriate significant other, and/or guardian.
Periodic Review: Review the course of opioid treatment and any new information about the etiology and impact of the pain. Adjust the treatment plan as necessary.
Consultation: Refer the patient for additional evaluation and treatment as necessary.
Medical Records: Keep accurate and complete records. Records must remain current and be available for review.
Comply with Controlled Substances Law and Regulations: You must be licensed in the state of NH, have a valid controlled substances registration and comply with federal and state regulation for issuing controlled substances prescription.
